What is a Continuum of Care Community? Why is it important?
Continuum of care refers to a healthcare system that offers a comprehensive range of services to cater to a person’s evolving health needs over time. It recognizes that individuals may be most at-risk during gaps in their care and aims to prevent this by providing continuous support. At The Ohio Eastern Star Home, this approach is integral to our philosophy of care.
Imagine how much of a blessing an environment like this can be for couples! If one partner requires constant care, and the other is still independent, they can remain together on the SAME campus!
